I accept the risks inherent in travel in a foreign country by air, land and on horseback. I have been advised to obtain trip cancellation, and emergency evacuation insurance.  Initial_________ 

I understand that this trip takes place at elevations as high as 12,000 feet above sea level.  I understand there are risks with travel at such altitudes.  I understand the above trip takes place in rugged outdoor conditions (including camping) that contain inherent dangers due to weather, terrain, flora and fauna and distance from hospital facilities.  I understand there will be rustic accommodations, unsanitary conditions and unfamiliar foods.  I understand I have the option to retire from the trip if I feel unsafe.  I further agree to bear the expense of voluntarily retiring from the trip before it is concluded.  Initial___________ 

 I understand that riding horses is inherently dangerous and those dangers include risk of injury and death.  I understand those risks are increased when riding unfamiliar horses and using unfamiliar tack.  I also understand the risks involved in riding horses cross country, on rough roads and trails.   I agree to accept full responsibility for any injury suffered while riding on and working around the horses and tack on this trip.  I have been advised to wear a helmet while riding and accept the risk of riding without a helmet if I so choose.  Initial__________ 

I certify that I have spoken directly with Boojum Expeditions staff  about the nature of this trip and have had my questions regarding all aspects of the trip including risks answered to my satisfaction.  I understand that Boojum Expeditions acts only as an agent in arranging transportation, meals and accommodations and auxiliary services such as the use of horses and camping gear. In the event of a dispute with Boojum Expeditions resulting from participation in the above mentioned trip I agree to resolve that dispute through binding arbitration under the rules of the American Bar Association.  Initial_____________
